MEDICO/LEGAL ADMIN SUPPORT

If you are requesting an assessment and report on behalf of your solicitor, the best means of contacting us is via email:

connect@nomchong.com.au

Download a copy of the Client Contact Details PDF, fill it in, save it with the name of the client and attach it to your email, when you make the request. You will be contacted via email (unless its urgent, in which case we will call you), and a list of available appointment times will be provided.

You can contact us via the office mobile - 0498 95 7777

Once an appointment has been booked for your client, we will email your client with details confirming the appointment and referring them to a link that explains the assessment process and what they can expect.   If you would like a PDF of that same information sheet - click here.

Appointments for the assessments are usually made at 9am on the selected day and the report is usually completed and available by close of business (C.O.B.) that day. Prepayment is required, preferably by EFT. Once the report has been completed, you will be sent an email requesting the report, with a tax invoice attached.

Once payment is received, the completed report will be emailed to you.  It may be password protected using a format of the date of birth of the client eg 00/00/0000 - including the /.  You will be advised on how to apply the password, in the email.

If you wish, a paper copy of the report instead, it will be dispatched in the post, when payment is received.

If you make a payment via EFT, please identify the payment with the patient's name and/or email us so that we can attribute the payment to the correct client.

Please note that the reason the appointments are made in the morning is so that the report can typically be completed by C.O.B. that day.  This reduces delays in the provision of the report once payment has been received and you can rely on having the report asap after the day of assessment.
